A Summary Care record is an electronic record containing key health information which can be made available to NHS Healthcare staff.
If you would like a summary care record you need not do anything
If you do NOT want a summary care record you can complete the opt out form (on this website) or call at the practice for a hard copy.
If you opt out you must give the form to the Admin staff at reception
If you are still unsure you can ask for a leaflet at reception or visit the website at www.nhscarerecords.nhs.uk
